We recommend contacting your insurance provider directly using the member services number located on the back of your card.

Here are some questions to help guide you: 

Do I have out-of-network acupuncture benefits/coverage through my policy? 

Does my policy allow me to submit documentation like an itemized receipt or superbill for reimbursement of covered benefits?

Are there restrictions on CPT (procedure) codes? If so, what are they? 

Is there a list of covered and non-covered diagnoses for acupuncture?

Is there a limit to the appointment of treatments my policy would cover? Does this fall under the same category as chiropractic or physical therapy? 

Where should I send necessary documentation in order to receive a reimbursement?

Is there anything else they can suggest in order for the reimbursement to go as smoothly as possible?
